Division of Intellectual/Developmental Disabilities (DI/DD)

The Division of Intellectual/Developmental Disabilities is part of the Colorado Department of Health Care, Policy and Financing.  DI/DD provides leadership for the state, however, on the website, information about services are integrated by the role of the person searching.  Here are a few links that we have pulled out that maybe helpful, but we encourage you to visit and search the complete website for more information.

You can find charts that compare the different Medicaid Waiver options for children and adults on the Long Term Services and Supports Training page.  These charts are helpful to find out who to contact for eligibility and other questions about Medicaid waivers.

 

Eligibility for programs is determined by different agencies.  You can contact the following for eligibility:

Colorado PEAK for eligibility for a wide variety of human services programs, including Medicaid.

Community Centered Boards for case management and services to adults and children with Intellectual and Developmental                Disabilities, including Medicaid waivers.

Single Entry Point Agencies  for case management and care planning for adults & children who qualify for Medicaid waivers.

Disability Determination Services for determination of Social Security (SSI and SSDI) eligibility.

 

Communication Briefs are published by the Division for Intellectual and Developmental Disabilities periodically.

This list of Committees, Boards and Collaboration offers stakeholders, including parents, a way to get involved with policy and decision making in HCPF.

SUPPORTS INTENSITY SCALE (SIS)
The SIS is a structured interview that is designed to identify the supports an adult with developmental disabilities needs to be successful in the world.  It helps to be prepared for the SIS Interview as that will determine the level of need and subsequent funding for an Adult with Intellectual and Developmental Disabilities seeking Medicaid waiver services.
