Eligibility for programs is determined by different agencies. You can contact the following for eligibility:
Colorado PEAK for eligibility for a wide variety of human services programs, including Medicaid.
Community Centered Boards for case management and services to adults and children with Intellectual and Developmental Disabilities, including Medicaid waivers.
Single Entry Point Agencies for case management and care planning for adults & children who qualify for Medicaid waivers.
Disability Determination Services for determination of Social Security (SSI and SSDI) eligibility.
Communication Briefs are published by the Division for Intellectual and Developmental Disabilities periodically.
This list of Committees, Boards and Collaboration offers stakeholders, including parents, a way to get involved with policy and decision making in HCPF.
Program Approved Provider Agencies are regulated through the Colorado Department of Public Health and Environment. You can find more information on facilities and provider agencies here.
SUPPORTS INTENSITY SCALE (SIS)
The SIS is a structured interview that is designed to identify the supports an adult with developmental disabilities needs to be successful in the world. It helps to be prepared for the SIS Interview as that will determine the level of need and subsequent funding for an Adult with Intellectual and Developmental Disabilities seeking Medicaid waiver services.
